Activity Description

Epstein-Barr virus (EBV)–positive B-cell lymphoproliferative disorders include a range of conditions, from benign, self-limiting diseases to aggressive lymphomas. Among these, EBV-positive mucocutaneous ulcers are localized lesions that typically arise in immunosuppressed individuals. These ulcers result from the interaction between EBV and memory B-cells, leading to localized lymphoproliferation. Clinically, they present as painless or mildly painful ulcers on mucosal surfaces, including the oral cavity, gastrointestinal tract, and skin. Most cases regress spontaneously or after reducing immunosuppressive therapy, though persistent or recurrent cases may require additional interventions, including radiation therapy, chemotherapy, or surgical excision. Understanding these ulcers' pathophysiology and clinical course is essential for timely diagnosis and management, mainly to prevent progression to more severe conditions.

This activity provides clinicians with the knowledge and skills to effectively identify, diagnose, and manage EBV-positive mucocutaneous ulcers. Participants gain an understanding of the pathophysiology, epidemiology, clinical presentation, and diagnostic approaches, including histopathological and immunohistochemical techniques. Treatment strategies are reviewed, emphasizing tailoring interventions based on patient-specific factors, such as immune status and underlying conditions. The course highlights the importance of interprofessional collaboration involving oncologists, immunologists, pathologists, nurses, and transplant specialists to ensure comprehensive care. By fostering teamwork, the activity supports early diagnosis, precise treatment, and the prevention of complications, ultimately improving patient outcomes and quality of life.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Differentiate Epstein-Barr virus-positive mucocutaneous ulcers from other conditions, such as malignant lymphomas or autoimmune diseases, based on clinical, histopathological, and immunohistochemical findings.

  • Screen patients with risk factors, such as immunosuppressive therapy or organ transplantation, for early detection of Epstein-Barr virus-related lesions.

  • Implement appropriate diagnostic protocols, including biopsy and Epstein-Barr virus-specific testing, to confirm the diagnosis.

  • Determine modalities to improve care coordination among interprofessional team members to improve outcomes for patients affected by Epstein-Barr virus-positive mucocutaneous ulcers.
