Activity Description

Transposition of the great arteries (TGA) is a congenital cardiac defect characterized by an embryological discordance of the aorta and pulmonary trunk. In dextro-transposition of the great arteries (d-TGA), the aorta arises from the right ventricle and the pulmonary trunk from the left ventricle, resulting in 2 parallel circuits that are incompatible with life unless there is a shunt, such as a patent ductus arteriosus (PDA) or septal defect, to allow for blood mixing. Congenitally corrected TGA (ccTGA) features both atrioventricular and ventriculoarterial discordance—a condition known as "double discordance." CcTGA may be associated with additional cardiac anomalies. Early diagnosis and surgical intervention, such as the arterial switch operation, are essential for optimal outcomes.

Evaluation of TGA typically includes imaging modalities such as echocardiography and magnetic resonance imaging to assess cardiac anatomy. Treatment options may involve surgical correction with an arterial switch procedure or palliation with a balloon atrial septostomy. Early diagnosis and prompt treatment are critical for improving patient prognosis. This activity provides healthcare professionals with an overview of the pathophysiology, clinical presentation, and evidence-based diagnostic and treatment approaches for TGA. This activity helps interprofessional healthcare providers strengthen their skills and knowledge by enhancing their ability to collaborate effectively as part of a healthcare team in managing TGA in patients.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify the clinical manifestations and diagnostic criteria for transposition of the great arteries, both dextro-transposition and congenitally corrected, to facilitate early detection and treatment.

  • Screen patients for associated anomalies and complications related to transposition of the great arteries, such as ventricular septal defects and pulmonary artery stenosis, to optimize treatment outcomes.

  • Select appropriate treatment modalities, including the arterial switch operation, tailored to the specific needs of each patient with transposition of the great arteries.

  • Collaborate with an interprofessional healthcare team to provide comprehensive care and coordinate follow-up for patients with transposition of the great arteries by managing complications and assessing long-term outcomes.
