Activity Description

Organophosphates encompass a diverse group of chemical compounds with common applications in pesticides and herbicides, as well as nerve agents in chemical warfare. When introduced into the body, organophosphates inhibit the enzyme acetylcholinesterase, resulting in an overabundance of the neurotransmitter acetylcholine. This surplus of acetylcholine in the body manifests with cholinergic toxidrome, which includes effects on nicotinic and muscarinic receptors, as well as the central nervous system. Respiratory failure resulting from bronchorrhea and bronchospasm is the leading cause of death in cases of organophosphate toxicity. Chronic toxicity and neurological complications, such as the intermediate syndrome, are also well documented. Although developed nations have experienced a decline in poisoning cases due to stricter regulations on the use of these chemicals, developing countries still face a clinical concern, especially when these chemicals are used for self-harm purposes. Antidotal therapy and comprehensive supportive care from healthcare professionals are necessary for the effective treatment of organophosphate toxicity to prevent morbidity and mortality. This activity reviews the assessment and treatment of organophosphate toxicity while highlighting the role of the interprofessional healthcare team in recognizing organophosphate toxicity and treating patients with this condition.

Target Audience

This activity has been designed to meet the educational needs of physicians, physician associates, nurses, pharmacists, and nurse practitioners.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify the clinical signs and symptoms indicative of organophosphate toxicity, including manifestations of cholinergic toxidromes, such as miosis, diaphoresis, and bronchorrhea.

  • Assess the severity of organophosphate toxicity using clinical scoring systems, such as the Peradeniya Organophosphorus Poisoning (POP) scale, to guide treatment decisions and predict prognosis.

  • Select the appropriate dosage and administration route for atropine and pralidoxime based on the patient's age, weight, and clinical presentation.

  • Collaborate with an interdisciplinary healthcare team, including emergency medicine physicians, intensivists, and toxicologists, for cases of severe organophosphate toxicity to ensure that patients receive timely and appropriate care in an intensive care setting.
