Activity Description

21-hydroxylase deficiency congenital adrenal hyperplasia is an autosomal recessive disorder caused by a deficiency of 21-hydroxylase, an enzyme responsible for cortisol synthesis in the adrenal glands. This condition is the most common cause of congenital adrenal hyperplasia, but it can sometimes be challenging to diagnose and treat due to limited awareness among general physicians, pediatricians, and the broader community. If undiagnosed and untreated, particularly in regions without newborn screening programs, this condition can significantly impact both the quality and quantity of life for affected individuals and their families. This activity reviews the epidemiology, pathophysiology, presentation, evaluation, and multidisciplinary management of 21-hydroxylase deficiency and its long-term impacts on the individual's physical and psychological health and well-being.

Target Audience

This activity has been designed to meet the educational needs of physicians, physician associates, nurses, pharmacists, and nurse practitioners.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Assess the magnitude of the burden of 21-hydroxylase deficiency, especially in countries lacking universal newborn screening programs.

  • Implement the current evidence-based standard of care pharmacological and surgical treatment, when indicated, in a timely fashion.

  • Evaluate and manage the long-term complications and comorbidities associated with this condition.

  • Collaborate with psychologists, pediatric surgeons, urologists, fertility experts, adult physicians, and support group members to provide multidisciplinary care, optimal quality of life, and a smooth transition to adult care.

  • Outline the pharmacologic therapy as it applies to 21-Hydroxylase Deficiency
