Activity Description

Drug-induced gingival overgrowth (DIGO) is a condition characterized by an excessive enlargement of the gingival tissues due to the adverse events of certain systemic medications. The most frequently implicated drug classes include anticonvulsants such as phenytoin, immunosuppressants like cyclosporine, and calcium channel blockers, notably nifedipine and amlodipine. DIGO arises when these drugs alter the metabolism of gingival fibroblasts, leading to excessive extracellular matrix deposition and tissue proliferation. This condition typically presents as firm, fibrotic gingival tissue that may obscure the teeth, causing aesthetic and functional challenges, including difficulty in maintaining oral hygiene and an increased risk of periodontal infections. While the severity of DIGO varies based on individual susceptibility and drug dosage, contributing factors such as oral hygiene status and genetic predisposition significantly influence its progression.

This course provides participants with in-depth knowledge about DIGO's etiology, pathophysiology, and clinical manifestations, along with strategies for diagnosis and management. Participants gain insights into recognizing early signs of gingival overgrowth, assessing risk factors, and implementing preventive measures such as optimizing oral hygiene and evaluating alternative drug therapies where possible. The course emphasizes the importance of interprofessional collaboration involving dentists, pharmacists, and other healthcare professionals to create personalized treatment plans. This team-based approach facilitates timely interventions, promotes adherence to care recommendations, and enhances overall patient outcomes by reducing DIGO's functional and aesthetic impact while addressing the underlying systemic conditions requiring medication.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Differentiate drug-induced gingival overgrowth from other causes of gingival enlargement, such as idiopathic gingival hyperplasia or inflammatory conditions.

  • Screen patients taking anticonvulsants, immunosuppressants, or calcium channel blockers for early signs of drug-induced gingival overgrowth to prevent progression.

  • Implement appropriate diagnostic protocols, including medical history reviews, drug interaction assessments, and oral examinations, to confirm drug-induced gingival overgrowth.

  • Communicate various interprofessional team strategies for improving care coordination and outcomes in patients with drug-induced gingival overgrowth.
