Duplex US can differentiate between acute and chronic venous thromboses. Acute DVTs are characterized by an enlarged non-compressible vein without collateral vessels whereas chronic DVTs have normal-sized veins with significant collaterals.
For at least the last decade, per the journal Circulation, the standard of care for assessing LE DVT is a complete color flow venous duplex study that examines both proximal (thigh) and distal (calf) veins. Old literature indicated US was insensitive for calf vein assessment, but with modern technique and equipment, this is no longer true.
