Identify patients at risk for chronic thromboembolic pulmonary hypertension based on clinical history, symptoms, and risk factors.
Develop a clinically appropriate diagnostic plan for a patient with possible chronic thromboembolic pulmonary hypertension.
Select individualized therapeutic interventions for patients with chronic thromboembolic pulmonary hypertension.
Implement effective interprofessional communication and coordination strategies to streamline care delivery and ensure comprehensive evaluation and treatment of patients with chronic pulmonary embolic hypertension.
