Activity Description

High-grade squamous intraepithelial lesion (HSIL) encompasses the conditions previously termed cervical intraepithelial neoplasia grades 2 and 3 (CIN 2 and CIN 3), moderate and severe dysplasia, and carcinoma in situ. HSIL is a squamous cell abnormality associated with human papillomavirus (HPV). Though not all HSILs will progress to cancer, HSIL is considered a precancerous lesion and is usually treated aggressively. The epidemiology, risk factors, and clinical presentation of HSILs are discussed. Participants will understand current diagnostic techniques, including cervical cytology, human papillomavirus (HPV) testing, and colposcopy, enabling clinicians to accurately identify and classify HSILs. Management strategies for HSILs, including both surgical and non-surgical approaches, will be addressed.

This activity also reviews the interprofessional team's role in managing this condition. Healthcare professionals will enhance their skills and knowledge in diagnosing and treating HSILs, ultimately improving patient outcomes and reducing the burden of cervical cancer.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Differentiate HSIL from other cervical lesions, such as atypical squamous cells of undetermined significance (ASC-US), low-grade squamous intraepithelial lesions (LSIL), and invasive carcinoma.

  • Apply knowledge of the histopathological and cytological features that define HSIL and the associated risk factors, particularly human papillomavirus (HPV) infection.

  • Implement evidence-based management strategies for HSIL while understanding the indications, benefits, and potential risks of the different treatment modalities.

  • Collaborate with the interprofessional team to ensure accurate diagnosis, appropriate treatment selection, and comprehensive care for patients with HSIL.
